Your completion of this form indicates you have read and agree to this Student Agreement. Violations of the agreement may result in losing the privilege of participation in the lessons program.

Student Lesson Agreement

1. Lesson fees are $30 per 30-minute lesson. Lessons longer than 30 minutes are at the discretion of the instructor. 

2. Full payment is due by the first of each month for that month’s lessons, unless other arrangements have been made. Checks should be made payable to FCCPS Band Boosters Lessons and mailed to FCCPS Band Boosters Lessons, c/o Julie Dorzab, 312 N Virginia Ave, Falls Church, VA 22046. 3. Parents are required to email or call the instructor a full 24-hours in advance of cancellation of a scheduled lesson. Payments for unexcused absences will be due in full. Lessons missed due to snow days, school closures, illness or legitimate emergencies may be made up without penalty at the discretion of the instructor. Make-ups are not required.

4. A two-week advance written (emailed) prior notice to the instructor and the lessons coordinator is required to suspend or stop lessons.

5. The lessons may take place at a time upon agreement between instructor and student and can take place either in person at Meridian High School, or online. All lessons must be completed by 9:00 p.m. Students and instructors should be ready in person, or to sign in, at least 5 minutes before lesson time. Scheduling of your child’s lesson is the responsibility of the instructor in coordination with the student and parent/guardian.

6. Students will conduct themselves in the same manner expected of them during school hours.

7. Students are expected to complete assignments given to them by their instructors and to follow the required practice schedule.

8. Students and parents agree not to hold Falls Church City Public Schools, Oak Street Elementary School, Mary Ellen Henderson Middle School, and Meridian High School or its agents, the FCCPS Band Boosters or its agents, or any other member of the Meridian High School community liable for anything that is not expressively stated in this agreement.
